It will take less than five minutes to complete and the responses we receive will help us improve how the Learning Disability team work with you and others in the future.

This survey can also be completed if you have been asked to share how you feel as part of our ‘Quality Assurance’ process. This will help us understand the difference we are making and how best to improve services for the people we support.
